Cornerstone Vitality ("Cornerstone Vitality," "I," "me" or "my") is a nutritional therapy practice owned and operated by Terri Ellis as a sole proprietorship in Texas. This policy explains what information I collect, how I use it and the choices you have. It applies to this website and to the services I provide to clients.

Information I collect

When you visit this website

This website does not use cookies, analytics, advertising trackers or contact forms. Like any website, the hosting provider (Amazon Web Services) may process technical information such as your IP address and browser type in order to deliver pages and keep the site secure. I do not use that information to identify you.

When you contact me or become a client

• Contact details, such as your name, email address and mobile phone number.
• Health and lifestyle information you choose to share, such as intake questionnaire answers, health history, eating habits, goals and session notes.
• Communications, including emails and text messages you send to me or receive from me.
• Scheduling and payment records needed to run the practice.

How I use your information

• To provide sessions, prepare personalized recommendations and follow up on your progress.
• To communicate with you about appointments, your plan and your questions.
• To send text-message check-ins, only if you have agreed to receive them (see below).
• To keep business records and meet legal obligations.

I do not sell your personal information, and I do not use it for advertising.

Text messages (SMS)

If you agree to receive text-message check-ins, I collect your mobile phone number and a record of your consent, and I store the messages we exchange so I can support you between sessions. Full program details, including how to opt out, are in the SMS Terms.

• No mobile information will be shared with third parties or affiliates for marketing or promotional purposes.
• Text-messaging originator opt-in data and consent are never shared with any third parties.
• Your phone number is used only to send you the messages you agreed to receive and to respond to your replies.
• You can opt out at any time by replying STOP.

Text messaging is not a fully secure channel. Please keep replies short and avoid sending detailed health information by text. When a message includes a secure link, use that link for anything private.

When information is shared

I share information only as needed to run the practice and only with service providers who handle it on my behalf, such as my email provider, text-messaging provider and secure data-hosting provider. They are permitted to use it only to provide their services to me. I may also disclose information if required by law, or with your written permission (for example, to coordinate with your healthcare provider at your request).

How I protect your information

I use reasonable administrative, technical and physical safeguards, including encrypted connections, access controls and multi-factor authentication on business accounts. No method of transmission or storage is perfectly secure, but I work to protect your information and limit access to it.

How long I keep information

I keep client records for as long as needed to provide services and to meet legal, tax and professional record-keeping requirements, and then delete or securely dispose of them.

Your choices

• You can ask to see, correct or delete the personal information I hold about you, subject to any records I am required to keep.
• You can stop text messages at any time by replying STOP, and stop emails by asking me.

Children

My services are intended for adults 18 and older. I do not knowingly collect information from children under 13.

Changes to this policy

If I update this policy, I will post the new version on this page with a new effective date.
